Not a good look, Bethesda. I will just hope that it was a mistake and it's something easily fixed, but I have very little faith.
Why would you blame the publisher when this isn't the case on any other platform?
Sony's licensing has lead to this before, not entirely sure if this is a Sony issue or a Bethesda/MS one. Doesn't feel like the kind of restriction MS would care to put on to PS users.

Maybe Bethesda is at fault. Maybe publishers and Sony need to communicate entitlements for upgrades.
This happened to tons of big releases that had a PS4/PS5 entitlement for the first year or so. Was a shitshow.
It's always fixed eventually, just gotta wait it out.

Nah, publishers choose this. Square Enix did it as well with FFVII Remake.

Likely the by product of a single individual bloody mindedly pushing through a particular setup without any proper forward thinking of planning - like main other PSN anomalies over the years.
Hopefully it's just a clunky PSN error and not a Resident Evil situation where Capcom just didn't wanna honour the PS+ entitlement

This is the reason why I have more faith in Nintendo (e.g. too show how low that bar is) than Sony when it comes to digital licenses. Such a cluster fuck over there on PSN.
The sad thing is... I got a non-Plus digital license in a bundle with my PS4, but because I claimed the Plus version I still can't download the PS5 version.
It's likely this. Sony themselves doesn't seem to prevent free PS Plus versions from being eligible for free upgrades, however due to the way SKU works on Sony's platform, publishers likely need to let Sony know in advance when they're letting Plus versions be eligible for free upgrades. It's very possible that most publishers just forget or not realize they have to do this.
Unless Bethesda decided that only people who bought the game get the upgrade of course.
Hopefully it's the usual issue but there have been plenty of games where it was the publisher's decision not to upgrade the Plus version.

Yeah I have the PS+ PS4 Fallout 4 in my library, but if I scroll down on the product page, it shows in a box "Fallout 4 PS4 & PS5, PURCHASED".

this is the same shit that seems to happen with other upgrades on Playstation- it's 100% on Sony and their convoluted upgrade systems.
I don't think this is that particular case. Sony has always had problems with licences for some godforsaken reason.
This is more likely a byproduct of how shitty their licensing issues are.
