Mai could not be included in Super Smash Bros. Ultimate without raising the game's CERO A rating states Sakurai

Evidently he still makes a joke pertaining to "good children" but it overall adds a little more context to the joke considering Sakurai's criticized CERO before due to the back-and-forth they went through having to change Palutena's cloth in the Japanese version of Smash 4 to appease their ratings.
Empathize with the localization team here since explaining what CERO is in an international video wouldn't really work.
Every Smash Bros. game was rated A in Japan, and as Sakurai says in the OP, that's the rating he seems to care most about keeping to.
Mai would probably coast just fine in the west since Smash is E10+ but CERO are extremely sensitive to any hints of nudity and they want that all-ages rating.
In all honesty that's just the card they were dealt with at the time, the rating should've been E10+.
And Smash UltimatE For Everyone outsold both in a year. They can advertise E rated games a lot easier to the masses.

Yep. Sakurai said that Smash 4 Wii U almost didn't make its schedule because of CERO. He was also baffled by Kid Icarus: Uprising getting a B rating due to Hewdraw's heads being knocked off though there's no blood or gore or anything of the sort.
That's an interesting question. I'm guessing the DLC comes with its own rating?

iirc Japan is very against decapitations. the Resident Evil games get decapitations removed over there.

Mai couldn't get in as a background character because Smash is for "good boys and girls".
I mean, it was obvious. During Wii U's development, Sakurai spoke about how difficult CERO was about avoiding panty shots in Palutena and Wonder-Pink.
In this game, it's clear from the alterations done to Mythra and Camilla.
Even other games were effected. Dragon Quest VIII on 3DS altered some of Jessica's alternate costumes to be less revealing from the PS2 version (people blamed Nintendo but it wasn't them).
Both Smash Bros and Dragon Quest strive for the A rating so they had to make adjustments. Apparently, CERO has gotten a lot more strict in the last decade.
It's funny really. I look at the titles that are CERO Z (their AO) and it's basically all Western titles with graphic violence. They treat gore the way we treat tits.

Well, yes. The heads just kinda pop off like anything else when targeting a weak point. There's nothing really special about them.

Terry's smash stage has like two dozen SNK character cameos in the background, but Mai is conspicuously absent, which Sakurai noted and explained that Smash Ultimate is a game play by good boys and girls of all ages, so she couldn't be added, and then you can hear off camera laughing

On one hand, they'd tone it down so that there's no blood or gore. Plus, he's just a guy in armor, nothing sexual.
On the other, I don't think they want younger people playing a Z rated game. Granted, Bayonetta is a D rated game but even that was pushing it.
It's not all that surprising, Dragon Quest 8's 3DS port famously altered assorted costumes and designs to get a CERO A rating. Any kind of high leg cut seems to be prohibited (the response is usually booty shorts or compression shorts)

I respect Sakurai decision of not even including her than covering her up and disrespecting the character design. Sakurai a man of culture. GO ALL IN OR NOTHING.
anyway at least this gives us proof that DLC does matter to the rating and they don't want to change it for japan at least

Of course. But in case there would be such an exlusivity deal, Sakurai would definitely not explain that such a deal exists and that they couldn't get her for that reason, he would rather make a joke like he did.

She's the Chun Li of SNK games. It's notable since 20 other SNK characters who aren't even remotely popular as her made it in, but she didn't. Not a big deal in the end, but it's not surprising that people are a little disappointed.
she's a major SNK character conspicuously absent from a stage that whose single defining trait is EVERYONE FROM SNK IS HERE, and also weird nerds are obsessed with their titty gremlins
