Immediately following the events of the ending to Metal Gear Solid V: Ground Zeros, Metal Gear Survive takes place in reality where after Big Boss's explosive departure, a mysterious wormhole opened up the sky above the burning mother base and pulled much of the base with it's debris and inhabitants, including a close friend of our protagonist who managed to escape with Big Boss. After the escape, our character falls into a coma for 6 months, eventually waking up in a facility owned by the Wardenclyffe Section, a secretive research organization of the U.S. government. Goodluck, an agent of Wardneclyffe, reveals the source of our coma is a mysterious alien parasite contracted from the wormhole, and sends us into a parallel reality beyond the wormhole in order to find a cure and save anyone trapped on the other side from whatever may lie beyond…
Players will find themselves creating and defending a new base of operations in this world while exploring the surrounding poisonous mist filled areas for food, water, materials, weapons, and most importantly, survivors. Opposing the players are the infected and mutated soldiers pulled in half a year prior, taking various forms to destroy our base and kill our protagonist and NPC's.
In the cooperative mode, players can work together with up to 4-players online to defend a resource extractor in Salvage missions, which provides valuable materials for use in either mode.
While defending the extractor, players will have to build and manage various structures and defenses in order to maintain control, while also encountering various side missions that can be used to get access to complete blueprints and new weaponry. Upon going to the cooperative mode, players will spawn in the pre-game lobby area which allows you to build new gear, weapons, and structures while also having an infinite inventory to test them out while you wait to start. Player's character's can also be upgraded in this area to take on specific roles in the cooperative multiplayer, such as specializing in melee combat or long range sniping on critical targets.

Is this a canon entry in the MGS universe?
No.
I heard you need an internet connection to play, even the single player. Is this true?
Yes, Metal Gear Survive does require you to be connected online at all times, even in the SP modes.
Is there a character creator?
Yes, pretty much identical to the one found in Metal Gear Solid V: TPP for reference.
Are there more cooperative modes besides the Salvage missions?
Won't know for sure until release, but we have yet to see anything else. That said, apparently the leaked achievement/trophy list suggests there might be more than what we have seen so far.
