There have been major hints, certainly - in particular, the TGS footage. But with as many actors involved as there are with this game, there was still a shred of doubt among the community. Whatever was left of that should be gone now.
Not at all. She and Kojima have teased through Twitter over the years. A woman with her appearance, in a body bag that featured the flag of the Netherlands, showed up in the TGS teaser. In addition, her manager has also alluded to her presence. This is simply icing on the cake.
Joosten is pretty rad. The ridiculous character design in MGSV worked against her entirely (yes, I'm aware of the attempt at explaining it) but I realized later on that the actress herself is cool. Hopefully she gets more to say and wear.
It's weird that they've been hiding this for so long. It's not really surprising.

Please edit this post if it's not cool, but I called the manager of Stefanie Joosten (Marc Truijen) and he told me Stefanie is not in any way involved with Kojima or Death Stranding. It definitely didn't seem like a cover up as he was honsetly quite surprised about this whole situation.
