As we have finally crossed the spooky threshold of October, my mind always takes me back to my favorite horror franchise in gaming this time of year: Silent Hill.

One of the trademarks of the series is the inclusion of creepy nurses who have no interest in your basic healthcare needs.
So I ask you, former vacationers of Silent Hill ERA, which style of damnation nurse unnerved you the most?

The originals. When first introduced, your experience with these nurses may vary. Mine was one of them lunging at me from the darkness, catching me off guard and generally confusing the hell out of me because unlike the other abominations of SH thus far, they seem oddly human... until you notice the twitching, tumorous growth on their backs. Sometimes you will find them simply shuffling around aimlessly in the black hallways.

Affectionately known as the 'Bubblehead Nurses', the immediate off-putting aspect of these nurses' designs is the mixture of the bulbous, faceless heads combined with the seductive nature of their body design. Reminding me of works from Clive Barker, it's an uncomfortable combination of disgust and lust. Their quiet nature and twitching heads, along with their preference of swinging pipes blindly, made them quite a bit different from their almost dream state, stabby predecessors.

In the third entry we get nurses that are fundamentally designed similar to the Bubbleheads (asset sharing? šŸ˜…), but this time with more human faces... with red squares around their mouths. Very Masahiro Ito-ish, if you are familiar with the more broad scope of his work. Also used a gun!

A shared design between the 2006 Christophe Gans SH movie and Double Helix's Homecoming, the nurse continues the 'scary/sexy' nurse trend, but in the case of the game- more advanced animation and slight changes in the design (nurse uniform appears to be made out of flesh and the faces are covered by fleshy bandages/flesh deformation).

*SH4 did not have a nurse. There was a highly disturbing Patient creature, which is sometimes confused as a nurse, but technically no. Origins has nurses, but they are basically the Bubbleheads with worse graphics. Downpour did not have nurses and Book of Memories and The Arcade just use pre-existing ones. I've added the Other option for those that may have only played those games.*

The rest are basically variations on "sexy nurse".

Be it the technical limitation or the gross growths on the nurses backs on SH1, they were a lot more creepy to me.

SH2... The sexy vibe fits (If you know the story) and the gauze wrapped face makes it look like the person you would regularly expect to help and assist you needs more help than you do. They are really well done.

Unfortunately, creators of the later games didn't understand why the nurses looked the way they did and went overboard, ruining them entirely.

Definitely the bobblehead nurses. Being seen as a sexy body with no face fits in context of the game and having just the head look fucked up kind of makes the whole thing extra fucked up if you get what I mean.

The nurses in SH3 may be the most disappointing thing about the game since it's just sex appeal. They should have looked more like the SH1 nurses but they went with something that doesn't fit or make sense. Although I find it funny how it unintentionally points towards Heather being into women.

Considering SH3's big twist, I'm a little disappointed we didn't get a PS2 update of the SH1 nurse.
That said, the doll face and red square mouth freaks me out in a way I can't really describe. They can also fuckin' SHOOT you, which made me more on edge in 3's Brookhaven than 2's.

Edit: hahah NightShift up there and I share the same disappointment!

While I mostly appreciate the narrative influenced design of 2's, I still can't take a hallway-full of nurses from the original. Those moaning, screechy sounds they make coupled with the growths/infection on their backs really messed with my 13-year-old self. Hospitals are never the same. Still my second favorite series of all time!

SH1 freaked me out most because I always thought the hump was a parasite controlling the nurses so you were being forced to fight real people unlike the monsters you faced up till that point

Played 2 first, so they naturally stayed with me for longer. I can imagine thinking 1 moreso if I had played that first.

Biggest disappointment of SH3 was the hospital. I mean yes it had some memorable moments (that mirror room), but it would've made far more sense to have Heather go to Achemilla instead. As for her version of the nurses....well gotta have them titty monsters I guess.

2 because I remember walking down dark hallways and they would come out of the darkness dragging their leg towards me lol. It also makes sense in that game to be sexy nurses.

I was going to say that I don't quite remember the SH3 nurses looking like a Suicide Girl or whatever but then I saw it's cosplay. Either way, SH2 ones bothered me the most. Reminded me of Jacob's Ladder or like Hellraiser creatures.

I was on my porn prowl the other day and my PornHub recommendations dead set had a video of a bloke laying pipe on women dressed as 5 of these things.

I would definitely say that's the most disturbing appearance I've encountered of this character.

Silent Hill 1, but I liked how the nurses in 3 had guns. Definitely caught me off guard during my first playthrough.

The Silent Hill 1 nurses. The combination of the dark oppressive environment, the sound (especially SH1's radio), the uncanny movements of the nurses, and their more human looking appearance made traversing the hospital in SH1 the most nerve-racking experience in the whole series for me.

Also, SH1's hospital had the most terrifying music ever:

Silent Hill 1 without a doubt.

You were fighting real humans controlled by a puppet parasite. In theory they could be saved, but the situation you're in forces you to defend yourself against them, and watch as they struggle, kick, and fight for their last breath when you knock them out.

It doesn't help that they groan as you hit them either, the male doctor dying moan is forever seared into my brain.

Hmmm... I certainly do not recall SH3 nurses looking anything like in the OP example. Yes, I see that it's a cosplayer, but as far as I can recall the SH3 nurses looked virtually identical to SH2 nurses.

The Silent Hill 3 nurses are odd. They're clearly supposed to be based on the SH1 versions, since they walk hunched over and look more like real people instead of disfigured monsters just like in SH1. But their model looks like it was a re-texture of the bubblehead nurse, so they end up looking like this weird blend of the SH1 and SH2 nurses.

1 or 2. I'd say I was more scared playing Silent Hill 1. I mean, on one hand, it's less convincing, a bunch of triangles, on the other hand, the roughness, the jankiness of the playstation, led to a creepier experience, and I was younger too.

Silent Hill 2 nurses were quite good though. 3's are fine, but not as scary. Everything beyond team silent flanderizes silent hill games into boring edgy sameness.

I think the Silent Hill 2 nurses. SH is one of my favorite horror game franchises of all time, and the nurses in 1 were definitely terrifying for their time, but I think, with the improved visuals, as well as the narrative theme in 2, the sequel nurses work so much better. They scared the shit out of me when I'd hear them shambling down the hallways, or get a hint of them moving around in the darkness, coupled with the radio static. Horrifying. Then they ruined them with each subsequent SH entry that featured them because the devs completely missed the point of why the nurses looked the way they did in 2, and thought "Hey, let's make them 'sexy' monster nurses cuz that's what they're supposed to be!" It was like cramming Pyramid Head into later games, despite his whole purpose being so closely tied to James' journey through Silent Hill.

With all of that said, if we were to get the Silent Hill 1 nurses actually rendered in high fidelity and matching their intended art design, they'd probably take the number 1 spot for me.

Since we're on the topic, I just want to say that if it wasn't for the stupid burping, the patients in Silent Hill 4 would actually be one of the scariest monsters in the whole series.

They look both disturbing and intimidating, they're really aggressive with their attacks, they take a lot of hits to kill, and, unlike many other monsters, they don't twitch or spaz around. They instead walk calmly towards you with surprising speed, similar to the children in SH1 or Mr. X in RE2. They have all the ingredients for an effective and memorable horror-game monster. Than the burping starts.

The original sh nurses in the movie were freaky. Gotta watch that again soon since it's Halloween month

Silent Hill 1 definitely. The realistic looking nurses hunched over being controlled by some kind of parasite is more unsettling to me.

Huh. I just watched some game play on youtube... the character animation is very similar to SH2 nurses, and I guess it's just so twitchy that I never got a good look at the face until now. Still, safe to say my recollection is pretty faded after all these years.

The OP misrepresents the SH3 nurses, IMO. They are still effectively freaky, and it doesn't necessarily need to imply that Heather is into women. It could represent that she's intimidated by sexy women, standards in society, etc. But in any case...

Isn't it just in SH2 that Silent Hill is a representation of the protagonist's psyche? I recall SH3 being someone else's representation of their inner demons.

I always theorized that Team Silent wanted to go to Alchemilla and Old Silent Hill in SH3, but didn't have the time/money to build the areas. Hence the large scale recycling of SH2 assets.
