Kojima understands spoken English very well, he just has some trouble speaking it. In a personal setting, I'm sure he wouldn't feel the need to have a translator.
His english pronounciation is iffy though. I suppose he just says "Mads..." and then Mads can tell what he's really saying by judging his expression.
